• NGB Form 34-1 - Application for Active Guard/Reserve (AGR) Program

• Record Review Rip (RRR) - available from vMPF

• AF Form 526 - Point Summary - available from vMPF

• Current copy of Fitness Assessment

• Medical Forms (DD 2808, DD 2807 and AF Form 522) - available from clinic

• DD Form 214 - Certificate of Release or Discharge from Active Duty

• Memorandum Stating Non-Eligibility for an Immediate Federal Retirement Annuity

    Medical:  

• AGR personnel due shots and blood draws can stop by the clinic Tuesdays 0900-1100 and Thursday 1300-1500

• The AGR medical point of contact is CMSgt Mike Garrett

    Military Finance Office:
